MEDICATION LIST
The physicians, nurses and pharmacists
at Huntington Hospital are concerned about
medication safety in our community. We would like
to help you make sure that you are receiving
the right medications in the right doses
at the right times. We would like
to help you be in charge of your own health.
Modern drug therapy provides many benefits
to patients, improving and extending quality
of life. But occasionally medications
can also harm, as when they are taken in
the wrong combinations, doses or times. Do
you see more than one physician? Many of
us do. When that happens, our medication
regimen may be altered by several practitioners – for
example, your family doctor, your cardiologist
and the orthopedist you just saw for knee
pain. Each physician needs to know
about ALL your medications. This includes
not only prescriptions, but also medications
that you choose to take “over the counter” and
even herbal remedies.
MY MEDICATION LIST is intended to help
you and your physicians keep track of all
your medications and any changes. We
encourage you to download this document. Write
down your current medications, including
prescriptions, over-the-counter drugs and
herbals. Remember that eye drops
and inhaled medications count! If
you have trouble, have a family member,
your physician, nurse or other healthcare
provider help you. And
if you need care at Huntington Hospital,
don’t forget to bring this list with
you. Bring it also to every healthcare
visit, so your physicians and nurses can
help you keep it up to date.
